Replacement loyalty cards.

Treat your Loyalty card as though it was cash. The Health Store does not accept any liability for lost, stolen or damaged cards or any points accrued on or redeemed from your Loyalty card without your knowledge.

However, if you email your details to info@thehealthstore.ie we will endeavour to find your loyalty card on our system and if successful we will reissue you with a replacement card and transfer the points from your old card as per our records.

If we cannot track your card we will send you a replacement loyalty card. Any points accrued on your misplaced card will be lost. Email your details to info@thehealthstore.ie
